The Community Health Nurse provides, individual, group and community health services/initiatives. Community Health Nursing involves listening to and working with people in local communities to maximize health potential and wellbeing through:

Eligibility

Eligibility for Community Health Nursing Services is assessed on an individual basis. Boundaries apply for some Community Health Nursing Services. The service can be accessed by calling the Duty Worker on the number below.

Fees

A small fee is charged for services. BBCHS fees are assessed according to Department Of Human Services guidelines.

Hours

Opening hours are Monday-Friday 8:30 am to 5:00 pm
