Chronic Disease
Cardiovascular disease, cancers and diabetes, along with mental health are presenting the most significant burden of disease in Victoria and the local communities, with frequently many service providers involved in the care of each client. Progression of these diseases and the need for hospitalization can be lessened by enhanced service coordination and improved management of chronic conditions, as well as early intervention and prevention strategies.
