PARKINSON'S SUPPORT GROUP

For people who live with Parkinson's Disease and their carers, to exchange information in a supportive environment. Monthly guest speakers on various topics.

HEALTHY LIFESTYLE LEARNING

This group aims to exchange information through discussions and speakers in a supportive environment, thereby enabling participants to make informed choices about the management of their health. There are also occasional outings. New members very welcome.

LIVING WITH DIABETES

A friendly information program for people with Type 2 diabetes or impaired glucose tolerance, to find out how to manage the condition and to prevent complications. The diabetes team, including a Die titian, Diabetes Educator, Podiatrist and Physiotherapist, will run the sessions. Partners or family members welcome at no extra charge. Afternoon tea provided.

FOOD FOR LIFE

This program focuses on practical advice on developing healthy meal plans, reading food labels, modifying recipes and making healthy choices when eating out.

PLANNED ACTIVITY GROUPS

Planned activity groups (PAG) are groups for adults with various disabilities and/or frailty to promote socialisation and provide respite for carers. They are only available after assessment to gauge suitability for each group.

CARDIAC REHABILITATION

This group is for people who have had a recent cardiac event (heart attack, angioplasty, bypass). It is an exercise program as well as providing information about heart disease. Requires referral from cardiac unit or local doctor.

CHRONIC FATIGUE SUPPORT GROUP

The group provides an information session with guest speakers. This is followed by a cup of tea for informal sharing of experiences.

CARERS' SUPPORT GROUPS

These groups are for people who are caring for others who are frail, disabled or suffering from a debilitating illness, including dementia. These groups offer mutual support, understanding and education with guest speakers assisting on specific issues.

'NEW HORIZONS'

Group offers 6 sessions for people who are no longer actively caring for someone at home. It is a Kingston Bayside Primary Care Partnership developed by Bentleigh Bayside Community Health Service, Southern Health and Central Bayside Community Health Service. The program explores issues relating to the past caring role, effects of caring on lives, provides emotional support surrounding loss and grief and effects with integration to community services with a focus on socialization.
