Mobility Support Service
TransAccess provides support for people who are frail aged and disabled and their carers to access appropriate transport options.
Volunteers are used to assist eligible people to travel on public transport or where appropriate provide a door to door service using program vehicles.
Wheelchair transport is available.
1. Eligibility
- Transport disadvantaged i.e are unable to access transport without support due to mobility, health, economic circumstance, language, social isolation or accessibility to transport options.
- Must reside in the local areas of Kingston, Bayside, Glen Eira, Port Phillip & Stonnington
2. Service priorities
- Priority is given to eligible people who reside in supported or low cost accommodation and receive a statutory income only.
- Priority is given to travel to Home and Community Care (HACC) funded activities
3. Cost
- All people transported by TransAccess (with the exception of Supported Residential Service [SRS] residents) are expected to make a contribution to the cost of transport.
4. Eligible transport requests
Transport to :
- social, recreational and leisure activities
- self help groups and carer support groups
- visiting friend and family
- educational and vocational courses/activities
- health related & therapeutic activities
- local connections, shopping & excursions
- group outings organised by an external agency where support staff are in attendance
5. Referral requirements
- referrals should be made at least 5 working days prior to the date of requested transport
- completion of the referral assessment process is a pre requisite to service provision
- all required information must be provided in order to receive a service through TransAccess
6. Service limitations
- Eligibility for service is not a guarantee of service
- Service delivery is subject to resource availability
- People who do not fall into the TransAccess priority group may not be eligible for all TransAccess service
